Current Families
Eligibility for ‘Current Family’ pricing requires at least one enrolled student in the most recently concluded or upcoming school year. (2022-2023 and 2023-2024)
Schedule Exceptions
Payment for Bay Farm Montessori Academy Summer Program is per-week and priced according to our programming schedule. Unfortunately, we are unable to accommodate partial week registrations and partially used weeks are non-refundable.
Last-Minute Schedule Additions
Adding weeks to your registration fewer than seven (7) days in advance is possible only on a case-by-case basis according to available staffing and capacity, though we will do our best to accommodate these requests.
Cancellation Policy
If a student needs to withdraw from a program for any reason, they must do so in writing (email) as soon as they have made the decision to withdraw. Correspondence must be sent to summer@bfarm.org. Cancellation must be made at least seven (7) days before the first day of your child’s registered week of camp. Any cancellation within seven (7) days of arrival will result in a charge of the total fee.
Behavior Policy
Bay Farm Montessori Academy reserves the right to withdraw a child for behavioral/conduct issues at any time for any reason. Payment for partial weeks will be pro-rata refunded to the nearest full day of unused time.
Medical Release
I understand that every effort will be made to contact me in the event of an emergency requiring medical attention. However, if I cannot be reached, I hereby authorize the personnel selected by Bay Farm Montessori Academy to transport my child to a medical facility and to secure for my child the necessary medical treatment as determined by the Emergency Medical Team. I understand the teachers in the program are trained in CPR and First Aid and I authorize them to give my child first aid when appropriate. I will hold Bay Farm Montessori Academy harmless in such events(s) except in case of negligence. Please note: According to Massachusetts law, we are required to transport an injured/sick child to the nearest hospital, which in our case, is Beth Israel Deaconess Hospital – Plymouth. We cannot honor any wishes to utilize another hospital in an emergency.
Privacy Release
I authorize Bay Farm Montessori Academy to print my name, address, and phone number in a directory for the use of parents to organize volunteers, set up playdates, mail invitations to, and/or arrange for carpooling. This directory is furnished for the personal use of Bay Farm Montessori community. Any use of this directory for solicitation or any other commercial purpose is unauthorized.
Photo Release
Bay Farm Montessori Academy has my permission to photograph my child and family during program functions (this includes video) and use these photos for program-related activities and marketing materials. I understand ‘family’ to include any and all guests I invite to program functions.
Release for Field Trips
Bay Farm Montessori Academy has my permission to transport my child to and from the program, during program hours, to specific destinations by modes of transportation that the program deems reasonable (cars with seat belts or buses). You will receive prior notification of each field trip.
Release from Liability
The undersigned as parent or guardian hereby voluntarily waives any claim for liability, personal injury, negligence, damages and choice of action against Bay Farm Montessori, Inc., its officers, directors, employees, agents, successors or assigns and assume the risk inherently associated with participation in the Bay Farm Montessori Academy Summer Program. I also acknowledge that I am fully aware of the risks associated with my child’s participation in the Summer Program and voluntarily accept those risks in exchange for an opportunity for my child to participate fully in those activities.
Sunscreen
As the parent or legal guardian of the registered child, I hereby give my permission to Bay Farm Montessori Academy Summer Program to apply a sunscreen product to my child, as specified below, when he or she will be engaging in outdoor activities. I understand that sunscreen may be applied to exposed skin, including but not limited to, the face, tops of ears, nose, bare shoulders, arms, and legs. I will apply sunscreen each day before my child arrives. I will send in sunscreen product(s) I would like my child to use. Bay Farm may choose not to apply sunscreen if I do not provide it.
Physical/Immunization
Please ask your doctor for a copy of your child’s physical/immunization form and provide this form to the program.
Copy of most recent physical must be received prior to the child attending camp.
Examples of ongoing health care issues would be allergies requiring medication, asthma, diabetes, etc. Please return this form to the office. Students who have ongoing health issues or take regular medication need to fill out an individual health care plan. A copy of this form can be found on the Bay Farm website.
